Q1) Management of Sanjeevani Hospital proposes to computerize patient register and billing. Visiting doctors visit the hospital as per the scheduled declared. The doctors are paid on monthly basis, against the charge-slips submitted. A detailed register is maintained, for patients admitted in the hospital. Room charges and Laboratory charges are recorded in the register, against every patient.

At the time of discharge, patient is provided the bill showing the above details. As a systems analyst-
(a) Draw E-R Diagram [07]
(b) Design a situation Level Diagram [08]
(c) Design a 1st Level Data Flow Diagram [05]

Q2) To record fixed deposit details, design a suitable screen layout with subsequent details:
(1) Name and Address of Depositor
(2) Amount of Deposit
(3) Rate and Period of Deposit
(4) Mode of Payment
State different validations and checks to be given. [10]

Q3) Consider the subsequent policy statement about commission for sales person.
If the quantity of sales is greater than Rs. 10,000 and advance collected is 50% or more, then he commission payable is 20% and if the advance collected is less than Rs. 10,000 but more than Rs. 5,000 the advance collected is 25% or more, commission payable is 15%. For any sales more than Rs. 5,000 the advance collected is less than 25%; then the commission payable is 13%. For sales other than mentioned above, no commission is payable.
Construct a Decision Tree for above case. [10]
